We are happy to submit claims for traditional-style insurance coverage. Please provide us with accurate insurance information at least one week prior to your visit. We will estimate your copay, and ask that any copay be paid at the time of your appointment.
PPOs: We participate with Delta Premier and PPO (including Michigan Healthy Kids/Delta Plan), BCBS PPO (and EPO), and MetLife PDP/PDP Plus.

We do not accept Medicaid or participate with any HMOs (DMOs) and cannot submit insurance claims with these entities.
